In-Home Services Assessment

Provided to all potential consumers of In-Home Services.  A qualified assessor conducts the assessment face-to-face in the consumer’s home. Results of the assessment are used to develop the Individualized Care Plan.  The assessment service is available Monday through Friday from 8:00 a.m. to 4:30 p.m., by appointment.
